CASE MANAGEMENT

AFC leads a system of coordinated HIV/AIDS case management, helping clients throughout the region access high-quality medical and social services.

AFC convenes the region's HIV/AIDS case management system, which is renowned as a national model in service integration. The system serves HIV-positive people who struggle with homelessness, mental illness, addiction, poverty, and other health concerns. More than 5,000 individuals receive assistance from the system's 165 case managers working at 49 agencies across metropolitan Chicago. Freddie Shufford, social services director at the Ruth M. Rothstein CORE Center, supervises case management services at Chicago's largest HIV/AIDS clinic. A past co-chair of AFC's Case Management Governance Committee, Shufford has helped improve service quality system-wide. AFC Program Director Román Buenrostro leads the region's case management system. He has consulted nationally on the development and evaluation of HIV/AIDS case management services, and is currently spearheading efforts to improve the collection and analysis of data reported by delegate agencies.


JANUARY 2004 AFC-funded case managers coordinate essential services for more than 900 homebound individuals with HIV/AIDS, allowing them to maintain their independence.

SEPTEMBER 2004 Nearly 600 HIV-positive individuals released from area jails and prisons are linked to medical care, housing, and transportation assistance through an intensive case management program.

DECEMBER 2004 Thanks to a partnership with the Pediatric AIDS Chicago Prevention Initiative and two area hospitals, all 28 HIV-positive pregnant women who received intensive case management services gave birth to HIV-negative babies.
