African-American Providers Team Up to Build a Stronger Community

The African American Housing and Health Collaborative (AAHHC) is a partnership of six healthcare, transitional living, and scattered site housing providers in the Chicago area. These providers are dedicated to simultaneously providing medical and housing services for HIV-positive homeless individuals.

The goal of the AAHHC is to build sustainable service capacity and infrastructure among African American community-based housing and healthcare providers to enhance the management, governance, and overall performance of the individual organizations. The short-term goal is to create a functioning collaborative model through monthly partner meetings and governance. The long-term goal is to work together collaboratively, supporting the work, resources, tools and staffing that is shared by all partners.
